• Section XI Misconduct- form for reporting to Section XI any player, coach or team misconduct.  Instructions for reporting a player, coach or school for misconduct are contained in  Misconduct Instructions and should be reviewed by officials before the season in case a report should become necessary.
• New York State Liability Form - form for reporting player injuries where the player is removed from the field for medical attention.  Fill-in the form and submit to Association Secretary John Kay
• Referee's Game Review Form- this form is for game reports.  This is a secure form for referees to report after varsity games.  After the form is submitted it will be e-mailed to the Interpreter for his records and a copy will appear on your computer for printing if you desire a record.
• Referee Development Evaluation Form - this form is for varsity officials to evaluate officials in the referee development program.
